Initial Consultation
Contact us at +61 434 302 750 to schedule an initial consultation. During this meeting, we will discuss your needs, goals, and how our services can support you on your journey.
NDIS Eligibility Check
We will guide you through the NDIS eligibility criteria to determine if you qualify for support. If you're not yet registered, we can provide information on how to apply for NDIS funding.
Planning Meeting
If you are eligible, we will assist you in preparing for your NDIS planning meeting. This involves gathering information about your support needs, goals, and any relevant documentation to present to the NDIS planner.
Service Agreement
Once your NDIS plan is approved, we will work with you to create a personalised service agreement outlining the services you will receive from Nurture and Care Solutions, ensuring clarity on goals, schedules, and expectations.
Start Receiving Support
After finalising the service agreement, our dedicated team will begin providing the agreed-upon supports. We will regularly check in to ensure that you are satisfied with our services and make any necessary adjustments to better meet your needs.
